For the final paper in this course, you will find a case study on a healthcare organization that conducted a quality improvement (QI) project where they implemented a quality improvement process such as those found in Chapter 4 of the textbook including, but not limited to: Shewhart Cycle or PDCA/PDSA Cycle, API Improvement Model, Six Sigma, Baldridge Award, etc. Once you identify the case study, you will include the following sections in your paper listed below.

You are required to submit your selected case study for approval. Please check the course schedule for the due date.

· Introduction/Background

· Background information about the healthcare organization

· The issue experienced by the healthcare organization

· QI Project

· Background information about the QI project implemented

· Identify the resources (inputs), activities (processes) and outcomes of the QI approach.

· What types of resources (staff, infrastructure, materials, information, technology, etc. were used for the QI project?

· What types of activities were involved? What was done? How was it done?

· What were the results of the QI project? How did it impact the health services delivered (if applicable)? Was there a change in behavior in the staff or by the patients? Was there a change in health status? Did it help to improve patient satisfaction?

· Were there challenges encountered?

· Lessons Learned

· What was learned from implementing the QI project?

· From your own perspective as a leader of the QI project, would you have done anything differently?

· What other recommendations would you suggest to tackle the issue?

· Conclusion

Paper guidelines

1. 4-6 pages

2. Have a cover page with the individual’s name, title of assignment, and due date of assignment

3. Include headers

4. Typed, using 12 point font, Times New Roman

5. Double spaced with 1” margins all around

6. Page numbers in the top right corner of all pages except cover page.

7. Use an APA format for citations and references. Make sure you give proper credit to the sources you are using. Plagiarism will NOT be tolerated.

8. Have a list of references in correct format at the end of the paper

9. Be error free – make sure you do a spell-check/grammar-check before submitting!

Failure to follow these directions (listed in #1-5) will result in an automatic 10 percentage point deduction from the assignment’s total point value.
