It is important that the community selected be one in which a CLC group member currently resides."},"content":{"rendered":"<style type=\"text\/css\"><\/style><p>My community is Bakersfield, California I was thinking of interviewing in regards to lack of mental health facilities and planning locally.<\/p>\n<p>&nbsp;<\/p>\n<p>This is a Collaborative Learning Community assignment.<\/p>\n<p>This assignment consists of both an interview and a PowerPoint (PPT) presentation.<\/p>\n<p><strong><em>Assessment\/Interview<\/em><\/strong><\/p>\n<p>Select a community of interest. It is important that the community selected be one in which a CLC group member currently resides. Students residing in the chosen community should be assigned to perform the physical assessment of the community.<\/p>\n<ol>\n<li>Perform a direct assessment of a community of interest using the \u201cFunctional Health Patterns Community Assessment Guide.\u201d<\/li>\n<li>Interview a community health and public health provider regarding that person\u2019s role and experiences within the community.<\/li>\n<\/ol>\n<p><strong><em>Interview Guidelines<\/em><\/strong><\/p>\n<p>Interviews can take place in-person, by phone, or by Skype. Complete the \u201cProvider Interview Acknowledgement Form\u201d and submit with the group presentation.<\/p>\n<p>Develop one set of interview questions to gather information about the role of the provider in the community and the health issues faced by the chosen community.<\/p>\n<p>Compile key findings from the interview, including the interview questions used, and submit with the group presentation.<\/p>\n<p><strong><em>\u00a0<\/em><\/strong><\/p>\n<p>APA format is required for essays only. Solid academic writing is always expected. For all assignment delivery options, documentation of sources should be presented using APA formatting guidelines, which can be found in the APA Style Guide, located in the Student Success Center.<\/p>\n<p>This assignment uses a rubric.
