He is exhibiting signs of depression.<\/p>\n<p>&nbsp;<\/p>\n<p>Client complained of feeling \u201csad\u201d<\/p>\n<p>&nbsp;<\/p>\n<p>Mother reports that teacher said child is withdrawn from peers in class<\/p>\n<p>&nbsp;<\/p>\n<p>Mother notes decreased appetite and occasional periods of irritation<\/p>\n<p>&nbsp;<\/p>\n<p>Client reached all developmental landmarks at appropriate ages<\/p>\n<p>&nbsp;<\/p>\n<p>Physical exam unremarkable<\/p>\n<p>&nbsp;<\/p>\n<p>Laboratory studies WNL<\/p>\n<p>&nbsp;<\/p>\n<p>Child referred to psychiatry for evaluation<\/p>\n<p>&nbsp;<\/p>\n<p>Client seen by Psychiatric Nurse Practitioner<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>MENTAL STATUS EXAM<\/p>\n<p>&nbsp;<\/p>\n<p>Alert &amp; oriented X 3, speech clear, coherent, goal directed, spontaneous. Self-reported mood is \u201csad\u201d. Affect somewhat blunted, but child smiled appropriately at various points throughout the clinical interview. He denies visual or auditory hallucinations. No delusional or paranoid thought processes noted. Judgment and insight appear to be age-appropriate. He is not endorsing active suicidal ideation, but does admit that he often thinks about himself being dead and what it would be like to be dead.<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>The PMHNP administers the Children\u2019s Depression Rating Scale, obtaining a score of 30 (indicating significant depression)<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>RESOURCES<\/p>\n<p>&nbsp;<\/p>\n<p>\u00a7 Poznanski, E., &amp; Mokros, H. Cleint tolerating well<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Decision Point Three<\/strong><\/p>\n<p>&nbsp;<\/p>\n<p>Maintain current dose<\/p>\n<p>&nbsp;<\/p>\n<p>Guidance to Student<\/p>\n<p>&nbsp;<\/p>\n<p>At this point, sufficient symptom reduction has been achieved. This is considered a \u201cresponse\u201d to therapy. Can continue with current dose for additional 4 week to see if any further reductions in depressive symptoms are noted. An increase in dose may be warranted since this is not \u201cfull\u201d remission- Discuss pros\/cons of increasing drug dose with client at this time and empower the client to be part of the decision.
