Recommendations should be evidence-based and from credible sources. The readings in module eight contains some suggested sources for obtaining health and screening recommendations for your patient.<\/p>\n<p>The plan for addressing the health promotion and disease prevention needs for your patient should include:<\/p>\n<p>Demographics:<\/p>\n<p>\u2013 Age, gender and race of patient<\/p>\n<p>\u2013 Education level (health literacy)<\/p>\n<p>\u2013 Access to health care<\/p>\n<p>Insurance\/Financial status<\/p>\n<p>\u2013 Is the patient able to afford medications and health diet, and other out-of-pocket expenses?<\/p>\n<p>Screening\/Risk Assessment<\/p>\n<p>\u2013 Identified health concerns based on screening assessments and demographic information<\/p>\n<p>Nutrition\/Activity<\/p>\n<p>\u2013 What is the patients activity level, is the environment where the patient lives safe for activity<\/p>\n<p>\u2013 Nutrition recommendations based on age, race gender and pre-existing medical conditions<\/p>\n<p>\u2013 Activity recommendations<\/p>\n<p>Social Support<\/p>\n<p>\u2013 Support systems, family members, community resources<\/p>\n<p>Health Maintenance<\/p>\n<p>\u2013 Recommended health screening based on age, race, gender and pre-existing medical conditions<\/p>\n<p>Patient Education:<\/p>\n<p>\u2013 Identified knowledge deficit areas\/patient education needs (medication teaching etc).<\/p>\n<p>\u2013 Self-care needs\/ Activities of daily living<\/p>\n<p>* The paper should be written and referenced in APA format and be no longer than 4 pages (excluding cover page and references).<\/p>\n<p><center><a href=\"http:\/\/onlineclassesguru.com\/orders\/ordernow\"><img decoding=\"async\" src=\"https:\/\/encrypted-tbn0.gstatic.com\/images?q=tbn:ANd9GcTyj99p60XCLyLk1htB7-1neRt8-2QdnenNlQ&usqp=CAU\"target=\"_http:\/\/onlineclassesguru.com\/orders\/ordernow\"\/><\/center><p>","protected":false},"excerpt":{"rendered":"<p>Please use the patient information provided below for this paper.
