Assessment task 1<br \/>\nAssessment Title In session Quiz<br \/>\nTask Description This is a ten question medication calculation quiz. Calculators may be<br \/>\nused. Students are permitted to use one (1) A4 sheet of paper containing<br \/>\nwhatever handwritten medication calculation formulae they wish to refer to.<br \/>\nStudents are required to achieve 100% competency before the end of this<br \/>\nsession. Students will get a maximum of three attempts at the calculation<br \/>\nquiz.<br \/>\nAssessment Due Date Week two tutorial<br \/>\nReturn Date to Students Week 5<br \/>\nAssessment Length 10 questions. 30 minutes<br \/>\nWeighting 0<br \/>\nAssessment Criteria This assessment will be marked as correct or incorrect for each answer<br \/>\nReferencing Style NA<br \/>\nSubmission In class quiz to be handed to tutor during tutorial 2<br \/>\nSubject Learning<br \/>\nOutcomes Assessed<br \/>\nc) Critically analyse the factors that are affected or affect individuals<br \/>\n2 | P a g e<br \/>\n3 | P a g e<br \/>\n2. Assessment task 2<br \/>\nAssessment Title Concept Map and reflection<br \/>\nTask Description Choose an individual who is living with a chronic condition who you have<br \/>\nknown. This may be an individual who you have cared for during your<br \/>\nworkplace experience or may be an individual who you know personally<br \/>\n(this can sometimes be confronting). Do not focus the concept map on<br \/>\nyourself. Your main or key concept should be easily identified as the<br \/>\nindividual and subconcepts should branch appropriately from main idea to<br \/>\na complexity of at least four (4) concept levels (subconcepts). Graphically<br \/>\ndescribe the relationships between your chosen concepts in relation to an<br \/>\nindividual living with a chronic condition. Use the map to connect concepts<br \/>\nand facts (nodes) and to demonstrate relationships between concepts like;<br \/>\ndeterminants of health; the impact of living with a chronic condition;<br \/>\nindividual personal likes dislikes aspirations and dreams; and<br \/>\nrelationships.<br \/>\nYour map should contain appropriate descriptive links between nodes<br \/>\n(short phrases or specific\/descriptive words describing the relationship<br \/>\nbetween concepts). Do not repeatedly reuse the same link; construct a<br \/>\nvariety of links that describe the unique relationship between specific<br \/>\nnodes and include specific examples that clarify the meaning of a given<br \/>\nconcept. Include cross-links which demonstrate the complexity for an<br \/>\nindividual living with a chronic condition.<br \/>\nIn your reflective commentary provide a description and reflection of the<br \/>\ndevelopment of the concept map. Do not simply describe the map.<br \/>\nConsider how undertaking this assessment task has impacted on your<br \/>\nunderstanding of what it is like to live with a chronic condition. Consider<br \/>\nhow the development of this assessment task has impacted on your<br \/>\nunderstanding of a person-centred approach to working with people living<br \/>\nwith chronic conditions.
