I cant wear closed shoes and I was fine until I started going to the gym.<\/p>\n<p><strong>HPI:<\/strong>\u00a0E.D a 38 -year-old Caucasian female presents to the clinic with complaint of pain, itching, inflammation, and yellow right great toe. She noticed that the toe was moderately itching after she took a shower at the gym. She did not pay much attention. About two weeks after the itching became intense and she applied\u00a0Benadryl\u00a0cream with only some relief. \u00a0She continued going to the gym and noticed that the itching got worse and her toe nail started to change color. She also indicated that the toe got swollen, painful and turned completely yellow 2 weeks ago. She applied\u00a0lotrimin\u00a0\u00a0AF cream and it did not help relief her symptoms. She has not tried other remedies.<\/p>\n<p>Denies associated symptoms of fever and chills.<\/p>\n<p><strong>PMH:\u00a0<\/strong>Diabetes\u00a0Mellitus, type 2.<\/p>\n<p><strong>Surgeries:<\/strong>\u00a0None<\/p>\n<p><strong>Allergies:<\/strong>\u00a0Augmentin<\/p>\n<p><strong>Medication:<\/strong>\u00a0Metformin\u00a0500mg PO BID.<\/p>\n<p><strong>Vaccination History:<\/strong>\u00a0\u00a0Immunization is up to date and she received her flu shot this year.<\/p>\n<p><strong>Social history:<\/strong>\u00a0College graduate married and no children. She drinks 1 glass of red wine every night with dinner. She is a former smoker and quit 6 years ago.<\/p>\n<p><strong>Family history:<\/strong>Both parents are alive. Father has history of DM type 2,\u00a0Tinea\u00a0Pedis.\u00a0mother alive and has history of\u00a0atopic\u00a0dermatitis, HTN.<\/p>\n<p><strong>ROS:<\/strong><\/p>\n<p><strong>Constitutional:<\/strong>\u00a0Negative for fever. Negative for chills.<\/p>\n<p><strong>Respiratory:<\/strong>\u00a0No Shortness of breath. No\u00a0Orthopnea<\/p>\n<p><strong>Cardiovascular:<\/strong>\u00a0Regular rhythm.<\/p>\n<p><strong>Skin:<\/strong>\u00a0Right great toe swollen, itchy, painful and discolored.<\/p>\n<p><strong>Psychiatric<\/strong>: No anxiety. No depression.<\/p>\n<p><strong>Physical examination:<\/strong><\/p>\n<p><strong>Vital Signs<\/strong><\/p>\n<p><strong>Height:<\/strong>\u00a05 feet 5 inches Weight: 140 pounds BMI: 31 obesity, BP 130\/70 T 98.0, P 88 R 22, non-labored<\/p>\n<p><strong>HEENT<\/strong>:\u00a0Normocephalic\/Atraumatic, Bilateral cataracts; PERRL, EOMI; No teeth loss seen. Gums no redness.<\/p>\n<p><strong>NECK:<\/strong>\u00a0Neck supple, no palpable masses, no\u00a0lymphadenopathy, no thyroid enlargement.<\/p>\n<p><strong>LUNGS:<\/strong>\u00a0No Crackles. Lungs clear bilaterally. Equal breath sounds. Symmetrical respiration. No respiratory distress.<\/p>\n<p><strong>HEART:<\/strong>\u00a0Normal S1 with S2 during expiration. Pulses are 2+ in upper extremities. 1+ pitting edema ankle bilaterally.<\/p>\n<p><strong>ABDOMEN:<\/strong>\u00a0No abdominal distention.\u00a0Nontender. Bowel sounds + x 4 quadrants. No\u00a0organomegaly. Normal contour; No palpable masses.<\/p>\n<p><strong>GENITOURINARY:<\/strong>\u00a0No CVA tenderness bilaterally. GU exam deferred.<\/p>\n<p><strong>MUSCULOSKELETAL:<\/strong>\u00a0Slow gait but steady. No\u00a0Kyphosis.<\/p>\n<p><strong>SKIN:<\/strong>\u00a0Right great toe with yellow-brown discoloration in the proximal nail plate. Marked\u00a0periungual\u00a0inflammation. + dryness. No pus. No\u00a0neuro\u00a0deficit.<\/p>\n<p><strong>PSYCH:<\/strong>\u00a0Normal affect. Cooperative.<\/p>\n<p><strong>Labs:<\/strong>\u00a0Hgb\u00a013.2,\u00a0Hct\u00a038%, K+ 4.2, Na+138, Cholesterol 225, Triglycerides 187, HDL 37, LDL 190, TSH 3.7, glucose 98.<\/p>\n<p><strong>Assessment:<\/strong><\/p>\n<p>Primary Diagnosis:\u00a0Proximal\u00a0subungual\u00a0onychomycosis<\/p>\n<p>Differential Diagnosis:\u00a0\u00a0Irritant Contact Dermatitis, Lichen\u00a0Planus, Nail Psoriasis<\/p>\n<p><strong>Special Lab<\/strong>:<\/p>\n<p>Fungal culture confirms fungal infection.<\/p>\n<p><strong>As an NP student, you need to determine the medications for\u00a0onychomycosis.<\/strong><\/p>\n<p>1.\u00a0According to the AAFP\/CDC Guidelines, what\u00a0antifungal\u00a0medication(s) should this patient be prescribed, and for how long? Write her complete prescriptions using the prescription writing format in your textbook.<\/p>\n<p>2.\u00a0\u00a0What labs for baseline and follow up of therapy would you order for this patient?
